SUPPLEMENTAL BENEFITS

ELIGIBLE EMPLOYEES

All full-time employees working 20 hours or more per week are eligible to participate in the supplemental benefits programs immediately upon reaching full-time status.  These programs may be entered into at anytime.
 

THE FAMILY PROTECTION PLAN

This program is a voluntary term life insurance policy for individuals up to the age of 100.  It is available for employees, spouses, children and grandchildren.  The plan includes critical illness benefit and is portable and very affordable.  This program is available for as little as $4 per week.
 

CANCER PLAN

The Cancer Plan provides coverage for cancer and 20 other specified diseases.  It includes a wellness benefit, initial diagnosis, hospitalization, physician, transportation, lodging, etc., extended care, new or experimental treatments, and other cancer treatment benefits. It includes an optional intensive care rider and a waiver of premium period after diagnosis.  This plan is available as individual or family coverage.
 

OFF-THE-JOB ACCIDENT W/ DISABILITY

This plan includes accident insurance that can pay you a lump sum benefit for off-the-job accidental injuries, plus some medical benefits including hospital and ambulance.  It includes benefits for dislocations, fractures, loss of limbs and death with a disability benefit for the primary insured only.  This plan is available for you or your whole family and covers an unlimited number of accidents.
 

DISABILITY INCOME

Supplemental disability income eases the burden when your paychecks stop.  As you recover, disability insurance can help you: cover bills, support your family and protect your hard earned savings.  It gives you a source of income when you can't work because of a covered sickness or off-the-job injury.  This plan includes a monthly benefit you can choose, up to 60% of your income.  It pays total and partial disability with multiple elimination periods to choose from.  You have a choice of benefit period up to 24 months and a guaranteed renewable plan up to age 70. The affordable rates are age banded and the policy is portable.
 

SUPPLEMENTAL HEALTH OPTIONS PLAN

The SHOP Package consists of a base Hospital Indemnity Policy with Intensive Care plus you can choose up to 9 health insurance options.  Your options include:  Initial Hospitalization - Outpatient Emergency Accident - Inpatient & Outpatient Physician Benefit - At-Home Nursing - Surgical - Transportation - Accident Disability Income - and Specified Disease Disability Income.

The SHOP Plan is available to individuals and their dependents and is offered in three packages:  Basic, Enhanced & Premier.  Rates vary by age and package.
 

HEALTHCARE & CONSUMER SAVINGS BENEFITS PACKAGE

This is a premium benefits package offered through a membership program.  This is not an insurance policy, but a Provider Access Organization that allows members "access" to a comprehensive group of benefits that include:

  • $7,500 benefit for Accidental Injuries ($100 deductible) - Premier+ Secure+
    $10,000 Accidental Death and Dismemberment - Premier+ Secure +
    Disability Benefits - Premier+
    Pharmacy and Mail-order Prescriptions - All Plans
    Dental - All Plans
    100% paid annual eye exam for each family member - All Plans
    Discounts in lenses, frames and laser surgery - All Plans
    Liberty Medical Program
    And much more.

There are 4 levels of membership including Premier+, Secure+, Access+ and Financial+.  Membership includes the primary member, lawful spouse and unmarried dependent children up to the age of 19 (up to the age of 24 if enrolled full-time in college).
 

PRE-PAID LEGAL SERVICES

This program is a legal services membership plan.  As a member, you have 5 core benefits available:  Preventative Legal Services, Motor Vehicle Legal Expense Services, Trial Defense Services, IRS Audit Legal Services and Other Legal Services.  The monthly membership fee is very affordable and allows you instant access to legal advise when you need it.  For more information please contact our Benefits Department at (405) 842-5900.
 

BANKING & CREDIT UNION SERVICES

AmeriResource offers direct deposit of payroll checks to employees working at locations that approve payroll a minimum of 48 hours in advance of payday.
